Challenges of Translating Metaphoric Language in Mental Health Counseling

Abstract— Metaphoric language is integral to how clients narrate distress, construct identity, and negotiate change in mental health counseling. Yet when therapy occurs across languages—through interpreters or bilingual practitioners—metaphors frequently become the fault lines where meaning fractures. This manuscript synthesizes insights from cognitive linguistics, translation studies, intercultural communication, and clinical practice to explain why metaphors are fragile in cross-lingual encounters and how these breakdowns affect assessment, alliance, risk appraisal, and treatment planning. We propose a mixed-methods framework combining corpus-guided elicitation, expert ratings, and a simulation study that models translation outcomes across metaphor types (conventional, embodied, idiomatic, and novel) and strategies (literal, sense-for-sense, paraphrase with gloss). Using a simulated dataset (N=1,200 utterances) spanning three language pairs (English↔Hindi, English↔Bengali, English↔Spanish), we estimate misinterpretation rates, entailment loss, and downstream clinical impact (risk escalation/deflation). Results suggest that literal renderings perform adequately for embodied metaphors (“a heavy heart”) but fail on idioms and creative metaphors; sense-for-sense and paraphrase-with-gloss reduce misinterpretation and valence drift—particularly in high-stakes content—when interpreters are clinically trained. We discuss implications for the DSM-5-TR Cultural Formulation Interview, propose an operational decision tree for translators and clinicians, and outline training, documentation, and workflow recommendations (pre-session briefings, metaphor journals, and dual-column notes).
