Abstract— Dietary change is foundational to the management of chronic diseases such as diabetes, hypertension, cardiovascular disease, chronic kidney disease, and non-alcoholic fatty liver disease. Yet, adherence to prescribed diets remains stubbornly low. A growing body of work suggests that language—in the literal sense of the words and symbols used, and in the broader sense of culturally anchored meanings—shapes patients’ understanding, motivation, and daily food choices. This manuscript synthesizes theoretical and empirical insights on how language affects dietary compliance across chronic conditions. Drawing on integrative review methods, it examines mechanisms spanning health literacy, language concordance and interpreter use, plain-language design, numerical comprehension, culturally tailored metaphors, framing, pictorial communication, and multilingual digital nudges. The paper organizes findings into a practical “LANG-CARE” framework—Localize terms and units, Anchor to culture and cuisine, Navigate numeracy, Guide with plain language and visuals, Co-produce with family/community, Augment with multilingual tech, Review with teach-back, and Enable with system supports. Methodologically, we outline an integrative review protocol and thematic synthesis approach that surfaces cross-cutting determinants, design features, and implementation enablers. Results from the synthesis indicate that (i) language concordance and plain-language materials consistently improve comprehension and self-efficacy; (ii) culturally and linguistically adapted diet counseling enhances intention formation and habit formation; (iii) everyday units (handfuls, katori, teaspoons) and recipe-level translations reduce cognitive load and support enactment.
